- Origination Fee - this is filled in in the top section under Estimated Fees payable by you to the broker - the Origination fee is entered at the end of the paragraph.
- All Estimated Fees. If any of these aren't applicable, enter N/A
- Referral Fees - if there is no referral fee payable, enter N/A
